NAT problems no inbound connections

When I do a Network Staus test I get this :

I get plenty of green smileys and IP binding is working perfectly using Express VPN but I get no inbound connections using any port. I use an Apple Airport Extreme router and I've tried everything. Could someone plese help!? Thank you, Dan.
